What we provide
Community Care Health Path, a brand of Prospect Digital LLC, writes plain-language guides to Medicare for the people who sign up, and sends text reminders to those who ask for them.
We are a private company. We are not Medicare, the Centers for Medicare & Medicaid Services, Social Security or any other part of the government, and none of them endorses us.
Information, not advice
Your guide explains how Medicare works and what your options are. It is general information, written in good faith, and not insurance, legal, tax or medical advice.
We do not sell insurance, act as an agent or broker, enroll anyone in a plan or recommend one plan over another. The choice of coverage is yours. Before you enroll, confirm the details with Medicare, Social Security or the plan itself.
Keeping it current
Medicare's rules, dates and costs change, often each January. We review our guides regularly, but something may change after we write yours. If you spot something out of date, tell us and we will correct it.
Your account
Please give accurate details and keep your account to yourself. You may sign up on someone else's behalf only with their agreement or legal authority to act for them.
You can close your account whenever you like by emailing us.
Prices
The guide is free and will stay free, and we never ask for payment details. No insurer or agent pays us.
Using the forms properly
Please submit the forms only for yourself, or for a person who has asked you to help them. Automated, bulk or false submissions are refused and may be blocked.
The text, design and code of this website belong to the operator.
Our responsibility
The website and guides are provided as they are. As far as the law allows, we are not responsible for coverage decisions you make or for losses caused by rules that changed after we wrote to you. Nothing here limits a right that the law says cannot be limited.
